Effective on or about May 12, 2022, Encompass Health Rehabilitation Hospital of Modesto will no longer provide supplemental social services or outpatient services. The elimination of these services will not result in a decrease in beds or personnel. The supplemental social services currently provide patients and their family with the means to understand and cope with the emotional and social problems affecting their health status. Once the service is eliminated, Encompass Health Rehabilitation Hospital of Modesto will ensure that patients and their families have access to comparable social services through contracted community providers. The elimination of this service will allow the hospital’s licensed clinical social workers to focus on case management services for inpatients and their families.  Patients will continue to receive necessary outpatient rehabilitation services by referral upon discharge from the hospital.

After the hospital eliminates supplemental social services, the three closest providers of social services will be:

• 1. Sutter Gould Medical Foundation Behavioral Health Center provides social services to Medi-Cal and Medicare beneficiaries. This provider is located at 3425 Coffee Road, Suite 2A, in Modesto, California.
• 2. Doctors Medical Center provides social services to Medi-Cal and Medicare beneficiaries. This provider is located at 1441 Florida Avenue in Modesto, California.
• 3. Center for Human Services provides social services to Medi-Cal and Medicare beneficiaries. This provider is located at 2000 West Briggsmore Avenue, Suite I, in Modesto, California.

After the hospital eliminates outpatient services, the three closest providers of outpatient rehabilitation services will be:

• 1. California Rehabilitation provides outpatient rehabilitation services to Medi-Cal and Medicare beneficiaries. This provider is located at 4660 Spyres Way in Modesto, California.
• 2. Andersen Physical Therapy provides outpatient rehabilitation services to Medi-Cal and Medicare beneficiaries. This provider is located at 1917 Coffee Road in Modesto, California.
• 3. Golden Bear Physical Therapy provides outpatient rehabilitation services to Medi-Cal and Medicare beneficiaries. This provider is located at 4318 Spyres Way in Modesto, California.
